 
                            Police are appealing for information following a fatal collision which occurred on the M5 on Tuesday 28 October.
Officers were called at around 11am to the southbound on slip at junction 29 following a report of a collision involving a Blue Ford Fiesta, Grey Land Rover Discovery and a Grey Mazda CX-5.
As a result of the collision a 94-year-old male passenger of the Ford sustained multiple serious injuries was taken to hospital. Police were informed that sadly he passed away on Thursday 30 October.
A second passenger in the Ford sustained a serious injury which is not life-threatening or changing.
The slip road was closed while an investigation was carried out at the scene.
Officers from the Roads Policing team would like to thank the public for their patience whilst the road was closed.
It is believed a number of people will have seen this but have been unable to stop due to it being on the motorway.
