Emergency services were called around 3.30pm after the black Audi Q5 and a cyclist collided on Bridge Road near to the junction with Countess Wear Road.
The cyclist, a man in his 70s from Exeter, suffered life-threatening injuries and was taken to Derriford Hospital.
The Audi driver, in his 70s and from Dawlish, was uninjured.
Police closed the road for around six hours to carry out an examination of the scene.
Officers are investigating the circumstances surrounding the incident and asking for anyone with information or relevant dashcam footage to contact them.
They are particularly keen to hear from the driver of a white Ford Transit van who assisted at the scene.
